Title: Maintenance Worker, Part Time Based in Trumbull County

Salary: $7.65 an hour

General Duties:

 1. General Maintenance:

  • Performs routine building and grounds maintenance as needed.
  • Picks up materials as needed.
  • Safety checks on agency vehicles.
  • Complete other duties as assigned by supervisor.
2. Responsible for quality control.

3. Serves as substitute van driver.

Qualifications:

Personal: Criminal background check; clean driving records; ability to deal with emotionally disabled clients and able to work independently to solve problems.

Job Classification: Direct Service Aide I

Title: Janitorial Supervisor

Salary: $7.65 /hr part time, pro-rated leave benefits

General Duties:

Supervises and educates client workers in janitorial services; performs janitorial services when required and is responsible for quality control through inspection of client's work; participates and plans with rehabilitation team.  Writes behavior reports, program notes, and department report; coordinates individual work duties to meet contract requirement.  Completes other duties as assigned by supervisor.

 

Qualifications:  Some janitorial and/or housekeeping experience; prefer one year supervisory experience; possess ability to deal with emotionally disabled client workers; be able to stand and walk for long periods of time; do occasional lifting of up to 50 pounds.  Must have reliable transportation to various work sites.

Pre-hire screening:  Criminal background, references, driving record, drug screen

Note: Good supplement to income for retirement or YSU students.  Majority of contracts on YSU campus.

Job Classification:  Psychiatric Nurse             

Title: Licensed Practical Nurse, Trumbull County         

Salary: Part time, pay rate negotiable depending on experience, primarily to work weekends and to fill in during the week for vacations and sick leave.

General Duties:

1. Interactions with clients are therapeutic and advance the goals set forth in the ISP.  Performance indicators:                          

    Medication Somatic services.

    Community Psychiatric Support Treatment.

    Spontaneous interactions and therapeutic activities.

    Teaching and consultation with clients in matters related to medication and medical issues.

2. Judgements made about clients or activities on clients’ behalf are appropriate both to the individual’s functioning and the agency’s risk tolerance standards.  Performance indicators: 

    Monitors compliance and educates on risks/benefits of such.

    Ongoing assessment of mental status and decisions regarding implementation of various services needed to develop and/or maintain skills essential to Recovery.

    Reviews available information prior to admission and during agency tenure to determine changes in service delivery/frequency as needed.

    Decisions regarding client’s medical needs in conjunction with Psychiatrist and Supervisor.

3. Medication management duties.  Performance indicators: 

Storage, handling, ordering and administration according to agency policy and generally accepted medical practice

4. Documentation is complete and timely.  Insures that sufficient medical referral information is available as soon as possible in order to determine frequency of services, baseline, medication changes. etc.

    Progress notes, billing logs, ISPs.

    Makes collateral contacts to gather medication information upon discharge to/from other facilities.

    Medication logs in client charts and required for medication room.

5. Documentation is clinically accurate and substantive.  Meets agency’s productivity standards.  Performance indicators:        

    Progress notes, billing logs, risk assessment and termination summaries.

    Billing logs refer to ISP.

    Doctor’s orders, medical problems, etc.

    Productivity as defined as hours of documented billable activity divided by total hours worked.  Standard for this job:.

6. Interactions with essential others is constructive and serves to advance the quality of care, the mission of the agency, and the agency’s standing in the community.  Performance indicators:

Peers and supervisors.

Other service providers.

Family and significant others.

Other concerned members of the community.

Interactions with physicians and other health care providers.

Qualifications:

Previous mental health experience preferred.  License:   LPN.

Miscellaneous: Criminal background check; CPR/First Aid Training preferred; TB Test; pre-employment drug screen, willing to work flexible schedule.

Title: Residential Monitor, Mahoning County

Salary: Part time, $7.65 per hour, .40 per hour shift diff/midnights

General Duties:

  1. Be aware of the whereabouts of residents and visitors during shift.
  2. Note adherence to program rules; maintain order.
  3. Log significant changes in resident's mode and/or behavior.
  4. Contact staff person on-call regarding unusual resident's needs and/or emergency situations.
  5. Transport residents for leisure activities and residents' errands deemed necessary by therapists and/or supervisor.
  6. Participate in leisure activities with residents, e.g. picnics, swimming, board games, volleyball, etc.
  7. When needed, help residents to stay "on task" with activities including but not limited to, dishwashing, cooking, chores, and laundry.
  8. Secure house and offices appropriately during scheduled shift.
  9. Answer telephone, taking messages when appropriate.
  10. Report any equipment malfunction in the log.
  11. Assist the rest of the staff in maintaining health standards.

Activities include, but are not limited to:

Helping with ex-resident's linens.

Cleaning up after accidents due to illness or other emergencies.

Education:  High School Diploma and training in human services.

Experience:  Training or experience in providing services to persons in a psychosocial rehabilitation setting preferred.

  2. Serve and maintain facility during evening, weekend, or overnight shifts.
  3. Log significant activity occurring during the shift and call professional staff regarding unusual circumstances as instructed.
  4. Answer telephone, take messages and under the direction of the Program Manager.
  5. Admit persons to the shelter as needed.
  6. Must be awake at all times during shift.

Qualifications:

Education: Minimum of high school diploma. College level training in human services field is preferred.

Experience: One-year experience in social services setting providing residential care.

Personal: Must be willing to work evenings, weekends, and overnight shifts.

General Duties:

  1. Provide on-site training to supported work clients.
  2. Assess behavioral strengths and deficits, implement behavior management techniques and collect progress data.
  3. Conduct quality and production ratings.
  4. Participate in client team meetings.
  5. Maintain regular documentation on client's progress via progress reports and other case file information.
  6. Establish working relationship with employers to foster employment opportunities.
  7. Gradually decrease amount of training and allow the site supervisor to assume supervision.
  8. Provide "awareness of disability" training to client's employer and co-workers as needed.
  9. Assess non-work related needs of client and provide information on available resources.
  10. Prepare summary reports to be sent to referral agent.

Qualifications:

Reliable transportation and cleaning driving record.  Most be willing to occasionally transport clients in personal automobile that is covered by at least the state minimum auto insurance. Openings in Mahoning, Trumbull and Columbiana counties.  At least two years education in human services. Adaptability, initiative, creativity, ability to plan and organize work and ability to work with adults who have barriers to employment.  Willingness to work a flexible work schedule.
